Registration

New Students to Benefield will need the following:

· Proof of residency

lease or house contract and a power (utility) bill with name and address on it.

If you are living with someone you need the following:

1. Residency Affidavit filled out completely ( pick up this at the school)
2. Lease or house contract from the person you are living with
3. Power (utility) bill from the person you are living with

· Birth certificate,

· Social security card/number,

· These two forms can be obtained from your physician or the Health Department

· Immunization Certificate on the GA Form 3132

· Vision, hearing and dental certificate on GA Form 3300

· Out of state records must be transferred to the Georgia Forms

· If you live in an apartment, we need your apartment number.
